When you first enter a new client into the system (therapist version), this is the entry form you use. It provides for recording a wide range of information about the client - or the bare minimum. The form contains a

series of tabs which organize the various kinds of information. The most basic information - name, address, contact information, etc., is on the first tab. You may wish to use only that level of detail. But the other tabs are there to provide for more detail, should you find it helpful.
